Explicit Consent

This text concerns the processing of the personal data you share through the two forms used in the online coaching process: the intake form, completed once at the start, and the weekly check-in form, repeated throughout. For the general privacy notice, see Data Protection Notice.

Personal data processed

Data collected in the intake form:

Name, surname and email address
Year of birth, biological sex, height, weight and body measurements
City / time zone and preferred contact channel
Training history, current program, weekly availability and equipment
Sleep, daily activity, stress level and general nutrition habits
Health information — health screening (PAR-Q+) answers, diagnoses, medications, surgical history, pain and injury information, pregnancy status. These are special category personal data under KVKK art. 6.

Data collected in the weekly check-in form:

Name, surname, email address and which week it belongs to
Weight, waist circumference and any other body measurements
Number of sessions completed, reasons for missed sessions, and per-exercise progress
Sleep, energy, muscle soreness, stress and daily step count
Pain report — the location and character of joint pain affecting training. This is health data and special category personal data under KVKK art. 6.
Progress photo — optional; you can complete the form without sending one. Photos are resized in your browser before they are sent, and location (GPS) and device information are stripped during that step. Purpose of processing

Providing the online coaching service
Preparing and revising a training program suited to you
Observing health constraints so the program can be applied safely
Conducting communication throughout the service

Health information is used only to observe constraints when selecting exercises and deciding loading. This is not a medical service; it does not involve diagnosis, treatment or rehabilitation, and personalised diet plans are out of scope.

Using progress photos for promotion is outside the scope of this consent. Photos you send through the forms are processed solely to plan your program and are not shared. If a photo is to be used in promotion, separate consent is requested by email for that use; the request states which photo will be used and where, and withholding consent does not affect the service. If you do consent, the photo is cropped so that your face is not visible, and you can withdraw your consent at any time, again by email.

Legal basis

The processing of health data always rests on your explicit consent. If you do not give consent you are not obliged to submit the intake form; in that case I will tell you that the service cannot be planned safely.

How data is collected and stored

When you submit the form, the information is sent to the server hosting this site, converted into an email there and delivered to the data controller's mailbox. No database record or file record is kept on the server; the form is stored only in that email. The mailbox is on the same server, so the form content does not leave the hosting provider.

Photos you send with a check-in follow the same path: they are delivered as email attachments, no separate copy is kept on the server, and they are not uploaded to any third-party service.

Cloudflare Turnstile is used for bot protection when you submit the form. Only verification data and your IP address are sent to that service; the form content and your health information are not sent to Turnstile.

Apart from these two, your form data is not transferred to any third party: no third-party form, analytics or mail service is used, and your data is not sent anywhere for advertising, marketing or data sales. Transfers to public authorities where required by law are reserved.

Rights of the data subject

Under KVKK art. 11 you may apply to the data controller and request the following:

To learn whether your personal data is processed, and to access it
To have it corrected if it is incomplete or processed incorrectly
To request its erasure or destruction
To request that these actions be notified to third parties it was transferred to
To object to the outcome of the processing and to claim compensation for damages

Consents collected in the forms

The consents ticked separately in the intake form are:

That this explicit consent text has been read and accepted
That the service is understood not to be a medical service and not to involve diagnosis or treatment
That no personalised diet plan will be provided
That in case of pain or injury, a medical assessment comes first

In the weekly check-in form only a single consent is requested, because the scope consents were taken once at the start: processing of the form content and, if sent, the photos, for the purpose of planning the program. The check-in cannot be submitted without this consent.
